Blood pressure and the DOT physical

Updated October 2026

High blood pressure is one of the most common reasons drivers get a shorter medical card.

How examiners often decide

Medical examiners commonly follow FMCSA guidance: readings under 140/90 can qualify for a card of up to two years, higher readings often mean a one-year card or a short card while you get your pressure under control, and very high readings can disqualify you until your pressure comes down. The examiner makes the final decision.

What you can do

Work with your doctor on a treatment plan, take medications as prescribed, and keep a log of home readings to bring to your exam.
